Medical Grant Application and Application Process

  • Where is the online medical grant application?
  • On this Web site there are examples of the types of medical services covered by UHCCF grants, such as speech therapy, physical therapy, wheelchairs, hearing aids, etc. What if our request is not one of these examples? Are there certain items that are excluded?
    • The Foundation excludes dental or orthodontic treatment that is unrelated to a serious medical condition. A serious medical condition that may be considered is cleft palate.
  • Is your Web site secure?
    • The online grant application is secure and uses 128 bit encryption.
  • How long does the application process take?
    • The time to fully complete the actual online application will take approximately 15-20 minutes to complete. The entire application review process will vary. If you choose to apply on behalf of your child, please submit an online application and forward us the requested paperwork as quickly as possible. Please read the apply page for more details.
  • I am a social worker or other service provider and I support a family that meets your criteria. Can I submit an online application on behalf of the family?
    • No. The child's parent or legal guardian must submit the online application.
  • Your guidelines require grant applicants to have "commercial health benefit plan ". Does that include federally funded or state funded insurance such as Medicaid, etc?
    • Please see the criteria for a helpful explanation of what a commercial health benefit plan is and is not.
  • I don't understand your guideline that states awards are generally awarded to families whose adjusted gross income per individual does not exceed $20,000. Can you clarify?
    • For example, if you are a family of three, awards are awarded to those families whose adjusted gross income (AGI) is less than $60,000, as indicated in the previous year’s Internal Revenue Service (IRS) tax return. There are additional application guidelines to meet in addition to the income guideline.
  • How do I know if my family meets this adjusted gross income (AGI) requirement?
    • You can view your AGI on your last year’s tax return. Here’s where to find it:

      • IRS 1040-EZ - Line 4
  • Can I submit a grant for past medical bills?
    • The UnitedHealthcare Children's Foundation requires a grant application to be submitted prior to the receipt of services. The Foundation does not pay for past medical bills.
  • How will you notify me if the grant application was approved or not approved?
    • You will be notified by letter.
  • If my child is approved for a grant, can I use the grant for any medical costs for my child?
    • The grant will only cover those items as outlined in the grant approval letter.

Contributions

  • Can I add the UnitedHealthcare Children’s Foundation to my employers list of charitable organizations they give to?
    • Yes. Please reference our name and tax identification number 52-2177891.
  • If I contribute where will my contribution go?
    • Your contribution will directly fund children’s grants in the region in which you reside, unless you specify that your contribution be used nation-wide.
  • What percentage of my contribution will go directly to children’s grants?
    • Thanks to the strong financial support of UnitedHealthcare and UnitedHealth Group, 99% of your contribution will go towards funding children’s grants.
  • Is my contribution tax-deductible?
    • Please seek guidance from your tax preparation specialist.
